6. The Process: Shared Care Agreement (SCA)
Once your medication is optimised, your psychiatrist will seek an SCA with your GP. The key benefit is that your prescription will be provided by the NHS, saving you private costs. If a GP refuses, we continue providing your prescription privately with 6-monthly vital sign reviews (free) and a yearly review appointment.

To ensure your safety and clinical appropriateness, the following conditions apply: Initial Requirements: Clinicians may consider initiating treatment if you provide a formal GP referral, your most recent Summary Care Record (SCR), and any relevant prior investigations. Safety & Investigations: If clinically indicated, we may require further investigations (such as blood pressure, heart rate monitoring, or blood tests) before starting medication. We will liaise with your GP to arrange these to ensure treatment safety. Additional Appointments: If your medical history is complex or if required health data is missing, a follow-up appointment may be necessary to discuss medication options in detail once all safety checks are complete. Note: Our priority is to ensure that any pharmacological intervention is both effective and safe for your specific physical profile.

Pre-Assessment Questionnaires
Complete brief questionnaires about ADHD symptoms, anxiety, and depression. Provide information from someone who knows you well and ideally from childhood. Takes about 1 hour total.
Diagnostic Assessment
Your psychiatrist reviews materials and conducts a 60-120 minute assessment covering current symptoms, childhood history, and life impact. Choose standard assessment or combined with treatment discussion.
Treatment & Medication (if applicable)
If medication is recommended, your psychiatrist prescribes through partner pharmacy. Regular 4-weekly reviews help find optimal dosing, typically requiring 3-4 appointments.
Ongoing Support & Shared Care
Once stable, we establish shared care with your GP for NHS prescriptions. Your psychiatrist provides annual reviews while your GP handles 6-monthly vital sign checks.
Long Term Medical Follow up
Once you are stable and under Shared Care, Eton provides structured 6-monthly follow-up to keep your treatment safe, effective and sustainable over years. Each review covers physical safety checks, medication optimisation, life changes (hormonal, occupational, relational), relapse prevention and forward planning — so your care evolves as your life does.

During Your Assessment
Your psychiatrist will ask detailed questions about your current ADHD symptoms and how they impact your daily life: work, study, relationships, and home activities. The assessment covers:

- Childhood History: ADHD symptoms present before age 12, school performance, and behaviour patterns

Assessment Options
Standard Diagnostic Assessment (60-90 minutes): Comprehensive diagnosis with separate treatment consultation if medication is discussed.
Assessment with Treatment Discussion (90-120 minutes): Combined appointment covering diagnosis and detailed exploration of treatment options including lifestyle modifications, psychological therapy, and medication.
After Your Assessment
Within 2 weeks, you'll receive a detailed written report explaining the findings and recommendations. This report can be shared with your GP, employer, or educational institution as needed.
If additional information is occasionally required to complete the assessment, a free feedback appointment will be scheduled.

Please note: this appointment is a medication review and transfer assessment, not a guarantee of a prescription. Whether we can take over prescribing depends on the quality and completeness of your previous assessment and your clinical suitability. If medication is started you will at least need one follow up after starting medication.
